It says he just had a genioplasty, but it kinda looks like he had more done.

Definitely a nose job. Also jaw surgery, you can see the screws. Imagine before is right at the beginning of treatment, prior to braces, and after is at the very end with braces removed.

Thats the thing, I thought it was lower jaw surgery too, but the placement of the screws dont match up to any of the other x-rays I can find.
I dont know if they are screws tbh.

Looks like upper and lower jaw surgery plus sliding genioplasty. I can see the screws. Nose only changed I believe because of the jaw changes.

what kind of screws go vertically from above the top teeth to below the top teeth in a straight line like that? If they are screws the placement doesnt correspond to any other jaw surgery x-rays.
I agree with the rhinoplasty, and he probably had extractions and the protrusive teeth brought in. Plus the genio obviously.

It looks like he had genio alongside a lower jaw advancement, a slight rhino to remove the dorsal hump. I dont think any lipo beneath the chin.
He might have had some screws/plates removed

I just saw my ceph xray this morning, screw placement is same as this guy's. The xray is a 2D image of both sides of the skull. It's actually 3 screws either side of the skull, but gives the illusion of 5 screws vertical.
The description indicates that a genio was performed to correct sleep apnea issues, it does not specifically say that was the only work done. Misleading, but the xray tells the full story.
That jaw was surgically moved.

Figured it couldnt just be a genio. Thanks for clearing it up.
