Advice w/ Jaw Surgery & Insurance

The first time I got them was from 3rd-5th grade, and the orthodontist recognized then that I would likely need corrective surgery in the future. We waited for my jaw to finish growing and continued with "phase 2." A year ago, almost to the day, I got braces put on for the second time in my life.

My surgery was approved by my parents insurance in December. However my family changed insurance in January when my dad moved jobs. The new insurance company (United Healthcare) declined the first time for "cosmetic" reasons, which I understand is fairly common. However, they recently declined a second time citing some reason in the fine print.

Looking for any advice on how to move forward. I'm meeting with my orthodontist on May 9th to discuss "other options." Both my orthodontist and orthognathic surgeon agree that it is medically necessary for me to get this surgery... How do I convince the insurance company the same?
