I can not make sense of this topic. earl25 is the only one on this forum who has had anything done that is even remotely similar to a Le Fort III. It's an extremely rare procedure and there are very few surgeons that have experience with it and even fewer who are willing to perform it on non-syndrome patients.

I'm not trying to be rude or anything. I'm just wondering how many people are considering getting the procedure and trying to find a surgeon willing to do it. I understand that it's very rare.

Even if you come to the conclusion that a Le Fort III is what you need for the most optimal result then you're still left with the problem of actually finding a surgeon that agrees with you and is willing to perform the procedure. I would consider the procedure if it was actually feasible to have it done without having to go to the other side of the globe and pay a small fortune for it.

so I found for him surgeons who can do that (as they claim)   and probably even if you are non-syndrom patient, dependly of how much your problem is visible and how it bothers you

Prof. Giorgio Iannetti
both from Rome (Italy) so it is possible to make 2 consultations during one short trip

They are cheaper than surgeons from Switzerland, especially one of them

Terry: write them, ask.


PS why a surgeon may not want to perform such surgery if a patient is non-syndrom ? if patient insists and really have visibly flattened face..... / recessed maxillas / cheekbones...... who knows.

I am.  Even though I had a Le Fort I 3.5 months ago and my surgeon has done LFIII on non-syndrome patients in the past and refused me both LFIII or a ZO since my face is "too wide".  But my orbits are flat and recessed.  LFI changed little under my eyes.

IMO, Europe would have the most surgeons doing it.  I think European max facs are at the leading (or bleeding) edge.

@plosko - So in your case would they have to do another lefort I to bring the lower jaw back since they're are moving the upper maxilla and zhgos forward?

Also did you find any surgeons willing to help you in North America or Europe?

No, I don't believe so.  My bite is perfect now.  My maxilla is where it should be, but the upper mid face  looks "left behind".

I think generally you should be interested in Italian surgeons.  I talked to girl who was born without zygoma bone on the one side and without whole or part (probably part) of facial nerve...  One of the italian Surgeons 11y ago managed to do nice work.. rebuild this etc and she looks nice - normal... assymetry due to paralized side of the face was compensated with some plastic procedures..and she smile normally . I saw her photos. She has damaged facial nerve and she smile normally :)

This surgeon is named Prof Sesenna, also well-known is Prof. Nocini.  I think many sugeons on the world should know them. Make your own researches. Ask your surgeons about them....

Perhaps Italian has something more in hands... ?   you know Ferrari, Lamborghini ;) maybe there is something......

That is the situation earl25 was in. His bite was good but the midface was recessed. Many class III patients will end up like that if you just do a standard Le Fort I without addressing the midface deficiency. earl25 ended up having a modified Le Fort III where I assume the surgeon cut above the maxilla so that the bite was not affected. I've never heard of that being done before though and earl25's surgeon said that he had never done a cut like that before.

Italians have a superior sense of the aesthetic.  Maybe Catholicism has something to do with this (compare Prussia with Bavaria, France and England), but that doesn't explain Poles.  ;)

While many class III patients would benefit from a  Le Fort III, often underbites cannot be fixed with just an advancement i.e. the upper jaw needs to be yawed, moved down, etc., to achieve an ideal bite.  This requires a combined Le Fort I + Le Fort III, which is what the Obwegesser girl had... or two separate operations.
