Surgeons who do revision genioplasties

I know that this is a really risky procedure no matter what, but I'm really not liking my chin implant because it looks boxy and unnatural. I kind of dread having my photo taken because my current implant looks almost cartoonish and very sharp on the corners. I got it done 4 years ago before I knew what jaw surgery was; had I known, I would never have gotten it :/  Does anyone know of some surgeons who are good at chin revisions?  I'd like to minimize scarring and get a feminine shape with more projection. I might wait until after surgery to have it done just to be on the safe side, so the surgeon doesn't have to be an awesome jaw surgeon or anything. Unfortunately I don't have the option of going to my original plastic surgeon because I signed a pre-op contract that said I can't get a re-do (for free) after 2 years. I don't know if it makes a difference but I am 19 and have pretty good skin elasticity.

I'm not really expecting many replies because I've searched endlessly to no avail. But I'm keeping my fingers crossed. Thanks for reading.

you had a genioplasty (silicone chin implant? i'm assuming) done when you were 15?

Dr Joe Niamtu and Michael Yaremchuk do revision chin implant surgery but they are located on the other side of the country

If you're currently in braces and getting ready for jaw surgery, I'd advise you get the implant removed, live "without a chin" for a few months which would actually be ideal since your tissue & nerves need to heal and just wait for your surgery

was your implant secured in-place with a screw?

i would remove it during orthognatic surgery!!! isn't that an option?

Yes, silicone along with a rhinoplasty. I don't think there was a screw. Almost 100% certain. I think my surgeon used just a natural pocket because it shifted a bit. I don't regret the rhinoplasty, but I probably should have waited on the implant. Idk... I do see your point, but I'm really afraid of how it will look. It looked bad before and it will only look worse once it is removed :/

Thank you so much for the recommendations! :D I had heard about Yaremchuk but not Niamtu. Travel isn't really an issue for me if it means a good result. I also found this guy: Dr. Wittenberg. Ah, that would be ideal! :)
Arnett wants to wait until after (he's really cautious about it and advises against doing anything to it, but he knows how unhappy I am with it), Cohen wants to do it during surgery, and I don't know what Schendel will say (I have a consult coming up in a few weeks). Both plans sounded good to me. I just refuse to remove it pre-op (at least more than a week anyway) :P

Ultimately, I just want the best jaw surgeon and best chin revision specialist. I wanted to get it done this summer, but  think it's a bit late.

I asked him about doing a sliding genioplasty during surgery and tossing out the implant. He said that it is risky because in most cases he has seen, even at the hands of skilled surgeons, there is almost always unattractive puckering and dimpling. I believe he used the words "horrendous" and "deformed". I liked that he was so straightforward about it, but it bummed me out to tell you the truth. Chin implants generally stretch out the skin and form a tough pocket of scar tissue around them. When they are removed, the tissue remains stretched out and not as strong as it used to be. Surgeons will either remove the scar pocket or leave it and sew the muscles back to where they should be. Cohen wants to leave it in because he said that it will shrink and go away on its own after a while.The chin won't ever be exactly the way it was pre-op, but it should provide support for the lip and chin pad (in my case I had virtually no chin pad).

My options as of now are:
a) leave the implant in and see how I feel about it post op (jaw surgery)
b) remove the implant and replace it with a sliding genio or a different implant during surgery
c) get the surgery and forget about replacing it

yeah arnett/gunson are fairly conservative when it comes to implants. there are risks but i would go for it. i dont know how much experience arnett has in confronting implants but i imagine he is competent.

dr. yaremchuk is one of the more well-known implant revisionists, id email him and send him your photos.
