Surgeons who won't accept out of network payouts?

I'm under a medical insurance that has limited health care providers, so I often have to seek out of network surgeons.
However, why is that many surgeon's won't accept out of network insurance payouts, even if there paid the full price to render there services, and my insurance is willing to approve out of network costs?

Pretty sure its because doctors who accept insurance for jaw surgery's have a much lower fee.
And usually aren't too experienced and won't be focusing on aesthetics like you will want.

The good/experienced surgeons with private practices charge much higher fees.
And insurance companies don't see any point in paying there high fees, when there are other surgeons (inexperienced) who charge much less.

I agree though, the insurance companies are wrong in thinking this, since jaw surgery is a complicated procedure.
Also it depends on your functional issues too in regards to insurance pay out, and some other factors regarding surgery.
