All, in all, to the best of my knowledge, distraction is a gradual process. So, you can observe it as it goes along and it would tend to be a 'wait and see' process regarding the 'maybe' of a later lefort 1. So, there's no need to think about the L1 right now. It doesn't seem like he's holding contingent that you have a lefort 1 later down the line if you get the distraction. The distraction alone would tend to make less the over cliff of the front teeth.  When the distraction period is over, he will have to remove the device and at that point you can decide to keep the residual overhang or go to the next surgery proposal to mitigate it more.

yeah and distraction could be a lengthy process. It will involve the distraction phase and then consolidation. The second is where the metal device (presumably in your mouth) would need to be in place anywhere from 6weeks to several months before he could proceed to the next phase of surgery. So I'd definitely ask him how long his whole process for you would take from finish to very end taking braces off all distraction equipment out of your teeth and teeth implants inserted (if done in bone, that also takes a while. Usually a few months with a metal stud, which, if it takes (a big if) can only be capped much later.

There are normally two devices what you can see on the link which I posted. One outer device for forward distraction, this stays 6 weeks after the last activation. And there is a second inner device for the elevation distraction, this stays 3 months after the last activation.

Are you sure that type of distraction device is reserved for younger patients who's jaws are still growing?

If you see the above link it, at least in these older cases looks like its a cut with fixed screws with the bone slightly flared outwards.
