She had some oral protrusion before the surgery. It's possible she had dental crowding or something like that causing the per-operative protrusion.
My guess is the nose tipped up from the forward movement of the maxilla. I can't really see her ANS on the scan, but it's possible that should have been shaved down to prevent some of this. She might have thin tissue that makes any movement more exaggerated as well. It's hard to say just looking at that.
Also, I wouldn't call that chimp lip. She looks better after even with those less than ideal changes.
