Thoughts on the 5 piece Mandibular Osteotomy?

I was curious if anyone had any insights on the 5 piece mandibular osteotomy. It seems to be seldom talked about as an option. Multi segmental lefort Is are quite common and widely accepted as an effective method for maxillary expansion but the same can't be said for mandibular expansion. Paul Coceaning (now suspended) and David Bell are the only surgeons I've heard doing this procedure.

What type of benefits would achieving mandibular expansion offer?
Why is it so rarely practiced even by the top surgeons in the world? Is it purely related to the difficulty positioning the condyles?
Do you expect to see this procedure become part of the orthognathic "tool box" for surgeons as the technology improves?
