Thank you. I figure the same but will vertical and horizontal lengthening be required? How is that analyzed?
I have seen a couple cases where 3 piece lefort was done for horizontal and vertical lengthening.

Sectional lefort is done for transverse discrepancies, not horizontal or vertical. Edit: perhaps used to flatten maxillary occlusal plane in some open bite cases? Not sure.
For aesthetic improvement, you will need genioplasty to reduce chin projection and/or CW rotation as part of whatever else is done, likely bimax advancement.

I would say you need some CW rotation because your OP is so flat, which will probably be enough to cancel out the need for a reduction genio. Bimax advancement since you have sleep apnea. Your case shouldn't be too hard.
