Title: What Kind of Surgery?

Here is a picture of me:
As you can see my jaw is not aligned properly, my nose is not straight either, and also one eye looks different than the other. What kind of surgery can fix all of these problems, My main problem is my angled smile and my eyes not being the same. If I had to get surgery to fix my nose then, I would do that too. If it helps I would be more than likely to go to the Mayo Clinic since I am somewhat near it.

I had braces so my teeth are straight, they originally gave me the option of jaw surgery but at the time my parents couldn't afford it. I had a huge overbite and overjet.
