Title: Can jaw surgery help TMD?

The problem I am suffering is not from pain, but the muscle spasms I get in my mouth from the decreased mobility of my jaw and the tightness in it.  Every time I talk with people, my mouth spasms and I have no control over it.  It is not because I am nervous or anxious.

This problem is literally ruining my life because I can't talk with people the way I should be able to.  I have a small jaw that seems underdeveloped and I am wondering if this could be the cause of all this.  Can orthognathic surgery help in my case?  I have tried a nightguard, muscle relaxers, and ibuprofen but nothing is working, and I'm getting desperate.
