Out of curiosity: Is this actually the fracture line of the type of Lefort III advancement Sinn also performs? Does he also perform the Lefort III including an upper jaw advancement as seen in this pic?
Reading a bit up on this stuff it could and change the orbital volume considerably. How does he make up for that?
Also seems like it would/could lengthen the midface somewhat. So the mandible should be rotated? What about the fracture in the nasal spine, how's that gonna heal?

He does both, but the full blown lefort iii is really for syndromal cases where the nose requires lengthening. His cosmetic lf3 does not involvd the nose and both orbital rims are moved independently. The maxilla is not involved either - the bite does not change. Draw two circles under the eyes that stop just above the infraorbital foramen and that's the surgery.

Thanks. If I draw to circles under the eyes that stop just above the infraorbital foramen they don't include the lateral portion of the orbital rim/zygoma: what am I doing wrong? Does anyone have a drawing of the exact cuts for this modified Lefort III as people here have started to call it?
The cuts go up the orbital rim laterally - hence the incisions in the corners of the eye.
