Title: What kind of movement do I need?

Here's a picture if I can get it to work.  One doctor says CCW 3mm upper and 8 mm lower.
Another doctor said 8mm both jaws straight forward.  He did mention downgraft but not anything more specific. 

I'm trying to figure out what the difference would look like.  The surgery is for severe sleep apnea.  Thanks.

CCW is the gold standard for sleep apnea.

Well, it's not CPAP either. That just treats the symptoms and doesn't cure anything. We're discussing jaw surgery here.
