Title: Does Zarrinbal execute Infraorbital rim implants ?
Post by: maverik46 on June 07, 2017, 08:51:27 AM
Hi all,
I have an appointment scheduled with Dr. Zarrinbal for the 23 of June. I contacted him for chin wing procedure, but i wonder if he is a good choise to fix another my other big issue with my eyes. I would need for sure an inferior orbital rim implant, but maybe also lateral.
Moreover I have ptosis, and a little bulging Eyes that would need probably an Orbital decompression, but I know that for this last problems is better an oculoplastic.

I doubt it as he strongly advice against implants. But please let us know when you ask him.

On another thread someone said that he uses cartilage as an onlay for the maxilla to make it look more forward, so no reason why he can't do that for the infraorbital rim. StupidJaws said that he hates implants but putting cartilage on the maxilla IS an implant so maybe he is just against synthetic materials.

Does anyone have an example of what the results from this look like?

If he use cartilage, would be possible to do orbital decompression after putting cartilage on orbital rim ?

How it would look depends on the shape he carves but essentially it would look exactly the same as a medpor or silicone orbital rim implant, but it would resorb to an unknown extent.

Oh sorry, I was referring to the onlay for the maxilla you mentioned. Would it essentially create that "curved" lip look people get after Lefort 1?

Oh good question, no one knows what that looks like as far as I know.
Last year I thought it would be a good idea to put cartilage on that area so pretty cool to hear Zarrinbal does that.
Actually, thinking about it, couldn't it look strange if the maxilla bone is more forward but the upper teeth are left behind?

Is it lyophilized though?

lyo cartilage still resorbs

I knew someone who had it placed under eyes and said no risk of resorbtion.Maybe not adjacent to bone?

He does AUTOLOGOUS cartilage or bone graft under the upper lip to mimic maxillary advancement.  He avoids ALLOPLASTIC augmentation and strongly advises against it.

Well then their surgeon was lying to them, there is a maxillofacial professor in Australia who uses cartilage and bone implants for cheekbones and other facial implants, he is very open that there will be resorption so he actually does an overcorrection.

He does AUTOLOGOUS cartilage
Oh in that case it doesn't sound like it will be a significant augmentation

Yeah his surgeon has less than stellar reputation on this forum.

Ah Sailer? Yeah I know when someone brought up the fact that cartilage resorbs he got angry, not saying it's a bad material or that it will all resorb, maybe something like 20 - 30 % but it depends on the individual and their lifestyle.

I know the company that actually supply lyophilized cartilage in Europe, it is called "Tutoplast", so any surgeon you find willing to use it, you can tell them where to order it.

Id love to get a giant pile of dead person cartilage and have it all surgically put into my mug

Lol! Most people find it damn weird to use cadaver cartilage, personally I would prefer it to plastic (medpor and silicone are composed of plastics).
The Australian surgeon I mentioned can use the patients own rib cartilage or hip bone but would that provide enough augmentation?

He charges 20,000 AU dollars or pounds, sorry can't remember, for autologous cheekbone implants, is that a fair price?

i dont understand why one would waste money on infraorbital rim implants when lefort 1 seems to disguise them pretty well, unless ur female theres no point in getting them cause every male i see has some sort of deficiency but then again im from england

I don't see how. It would only fill out the paranasal area. The curved upper lip is due to the angulation of the nasal base and upper teeth. Because of your compensation, your upper teeth are tilted back, hence the convex lip.

HOLY TITTIES THATS OUTRAGEOUSLY EXPENSIVE.  like sailer level expensive.

i would actually have no issue having dead person cartilage or bone in my face.  problem is, they CAN get infected, since the DNA dosnt match yours, theres a chance the body will consider it a foreign substance and try and fight it off.  IMO it beats having a chunk of your hip or rib removed.  Problem is, with osteotomies, when theres an infection, its not as simple as removing an implant.

I thought it seemed very expensive but didn't know what price to compare it to, I would compare it to cheekbone osteotomy but don't know the price of that.
His name is Professor David David, he looks pretty famous.

Yeah although the infection rate is meant to be low it can still get infected.
