Title: Worst Lefort 3 result ever?

Saw this on the interwebs and was kind of shocked, the guy had a modified lefort 3

No orbital rim advancement but still a good result. Who was his surgeon?

He needed jaw surgery, nothing more. He had no exophthalmos before, so he ended up with enophthalmos after. I'm surprised the doctor published this result. I suspect some younger surgeons just tick an lf3 off their bucket list and never do one again unless it's a full blown syndrome case. And this was a mere 4mm advancement of the infraorbital rims! Unless you have really buggy eyes, it shouldn't be done. Too imprecise - like an  aggressive rhino on a mostly non deformed nose.

He's left with a crossbite and I don't think his mandible was set back enough.  His mandibular incisors could have been tipped labially to help accommodate further setback of the mandible.  It looks to me like a multi-segment Lefort would have been appropriate to address the crossbite.

Yes the bite is poor. I don't understand why this was even published. Makes the surgeon look bad.

I think he could use a genio reduction.
The soft tissue does tend to stretch when facial disproportions are exhibited.
