Dentists just basically look for employment opportunities in your mouth. There was an article in Readers Digest many years back that said over 70% of dentists are dishonest and just want to line their own pockets.

Max facs are now MDs as well (only really old ones are only DDSs).

What bothers me is this kind of fraud should be easy to detect for an insurance company. If some guy performs way more expensive procedures than the norm you should flag it and investigate. You wouldn't need a lot of manpower to catch these ridiculous outliers.
