Hi everyone,
I’ve always been insecure about my side profile. I’m also about 70kg overweight right now, so maybe this isn’t the best state to judge things, but I’ve felt this way my whole life. I don’t have any functional problems — my bite feels fine, no breathing issues, and I’ve been told my airway looks good. This would be purely for aesthetics.
I’ve attached my ceph x-ray, as well as a side profile picture.
Based on this, do you think I would benefit from a sliding genioplasty, lower jaw surgery, or anything else?
Also open to fillers or other plastic surgery options.
Any feedback on my case would really help.

Possibly chin augmentation. But I would focus on weight loss for some fat reduction for now. Plastic surgery options for fat lipo under the chin exist. But best to consider that if most doesn't go away with weight loss.

Thanks Kavan, do you see any issues in regards to my lower jaw ( mandible ) ? I know a ceph x ray does not tell the whole picture.

Can't say for sure because the ceph is too dark for me to see the soft tissue profile relative to the bone/teeth. Do what you can do about reducing the face, neck chin fat via weight loss and put the maxfax stuff on the back burner for now.

Looks to me like a CCW case, but I'd also suggest losing weight first. Might be able to get away with just the chin if you're willing to compromise a bit. Consult with at least 3 surgeons and see what they say. My impression is it is a legit jaw surgery case, but of course that carries risk to the nose, etc, so if you can get away with just the chin as a compromise it might be worth it. It might result in a deep labiomental groove, though (in theory that could be mitigated with HA or a graft of some type, if needed).

Thanks for the comment, what do you mean by CCW and what are you seeing in the x-ray that makes you suggest that? Any other issues like recessed maxilla or mandible? Just trying to learn

Counter clockwise rotation. So the lower jaw would come out as the upper jaw mostly remains in place (more like a rotation than a movement) with maybe some downward movement in the back that will rotate the front of the maxilla upward. It seems obvious to me that's what you need, but I think you should lose weight first to see how that improves things. Also your curve of spee and curve of wilson both look incorrect. So you'd probably need at least 1.5 years in braces.
