Title: OSA bimax, looking for an aesthetics sanity check on prelim VSP
Post by: creatime on October 19, 2026, 01:41:47 PM
Hi all, mid-20s guy. I just got my preliminary virtual surgical plan (3-piece LeFort, BSSO, genioplasty) and have a planning appointment Wednesday.

Album with my current natural head position photos and the plan: https://imgur.com/a/preliminary-plan-yQZmZKS

Why I'm doing it: airway/apnea (RDI ~15, minimum axial area ~60 mm², posterior airway space 2–3 mm, currently on CPAP) and to fix a maxillary/mandibular cant.

I care a lot about the aesthetics (don’t want to look worse after surgery). My sleep study was done at my skinniest and I’m hoping to get off of CPAP (can’t see myself using it for the rest of my life and still have symptoms with it). Airway comes first, so I'm not looking to reduce advancement for looks. Just want to know if anything looks off within that.

Mainly wondering: does this plan look good and make sense for my face? Anything you'd flag to raise with my surgeon?

Three specific worries:
1. Is this advancement likely enough to address the airway issues?
2. The flat occlusal plane after surgery (10.7° → 1.8°). My NHP seems tilted down quite a bit from SN/Frankfort, so I'm thinking it'll look less flat in real life than that number suggests, but I could be wrong. The report doesn't say what the angle is measured against.
3. Whether the genio (chin ~17 mm forward total and ~4.6 mm down) makes my profile slightly too concave or my chin too long.

Honest takes welcome, thanks!
