I think that most of the really high quality surgeons are recognized within their field and, as such, receive more than enough patients through orthodontist/dentist referrals. They don't need to advertise, nor do they want to. I'm actually more wary of the surgeons that do advertise and really try to get their name out in the public, by the same reasoning (that they don't get enough referrals based on the quality of their work). So I think these surgeons who are regarded by their peers to be excellent are generally "unknown" in the sense that they don't have their face on a billboard somewhere.
