Before and after BSSO

I think the lower jaw looks still recessed. A genio would probably deepen the sublabial fold. Better result possible with ccw rotation? Other thoughts?

you should have had premolars extracted in your lower jaw to create more overjet to get a larger advancement.

a premolar is 6-7mm wide, so you could have gotten 6-7mm more mandibular advancement which would have been spot on i think.

caveat: your bite would be worse than it is now, but you'd look better.

to be fair though: the surgery was a definite improvement looks wise and the function (bite) looks excellent on your ceph.

you could still get chin wing with a bone graft from the hip to fill the sublabial fold.

Thanks again for opinion. Compromising the bite and risking relapse is probably not worth it if similar aesthetic improvement is (hopefully) possible with chin wing.

Has your surgeon said a genio would deepen the fold on you or are you just predicting?
