My jaw is rather small. It sits really far back. Its always bothered me. I had braces when I was about 14 to fix "buck teeth". Then at age 29 had braces a second time. I finished those some months back, but I am still extremely unhappy with how my jaw looks. Its rotated backwards quiet a lot. The post-braces treatment made me look even worse in my opinion. The changes are subtle, but less desirable, my lower teeth were pushed out a little bit wider and this changed the convex/concave contours of my lower face.
I am 31 years old but my jaw looks like it belongs on a small child :\
Before undergoing braces for a second time I consulted with a maxillofacial surgeon about a maxillomandibular advancement with CCW. He provided cephs (but never gave me a copy), the ceph sketch I saw looked REALLY GOOD, and when he actually met with me in person he immediately rejected me as a candidate (despite thinking I was a candidate prior to actually meeting me). He basically said "I dont think I can make you look any better". He then recommended I contact a former colleague of his who does more "cosmetic" surgeries.
Basically, there is no reason for me to need surgery other than the aesthetics of it. My ortho did say my maxilla was "slightly retruded". But my teeth are aligned pretty darn good. I refuse any extractions (except my wisdoms are gone). My ortho mentioned I could get pre-molar extractions followed by a BSSO advancement. But because I didn't want extractions of any teeth thats not an option since with a BSSO I would have an underbite.
But despite my teeth lining up good I really dislike my profile. I hate how backwards it is relative to the rest of my face (steep mandibular angle, retruded jaw?). The thing is I have a well developed chin based on the labiomental sulcus depth, so a genioplasty is probably not a good idea as it will just make my sulcus awkwardly deep. I also dislike the assymetry in my jawline from the front. The right side of my jaw does not protrude out at the mandibular angle the same as my left side, when relating to the zygomatic arch lateral protrusion. Meaning my zygomatic arch on the right side of my face sticks out farther than my mandibular angle, but on the left side the two are in a better harmony with eachother. I would estimate the mandibular angle is like 3-4mm less protruded (laterally) than its corresponding zygomatic arch on the right side of my face. When I open my mouth, my jaw veers a fairbit to the left. I also have a very hard time equalizing pressure in my right ear when SCUBA diving, sometimes causing me to abort dives altogether because it wont equalize. I've always thought this had to do with my jaw, but when I spoke to an ENT about it he said that was "impossible" (but he also seemed arrogant/ignorant/set-in-his-beliefs).
Yes I know I need a rhinoplasty pretty badly, my nose has a pretty harsh deviation in it and also some minor hump. The maxillofacial surgeon I saw told me to get a rhinoplasty, not jaw surgery. lol needless to say I didn't really like him or his attitude. As I mentioned, the ceph sketch he did of the possible outcome of a CCW and showed me when we met looked fantastic, but he only let me see it for a few seconds then snatched it back and said what he said.
What do you guys think?
Sorry I do not have a copy of my profile x-ray. All photos and X-ray shown are taken BEFORE my second bout of braces. Although the changes were minor, $7000 dollars and 2 years of my life later for virtually no difference from before :\
