Is this something I should only see the specialist for? I have one option in terms of that but it will cost exponentially more time and money as well as much higher risk as Alfaro is in Spain.

I'm wondering if it is worth it to go through the trouble of seeing him or is there any North American surgeons who can perform it.

Do you want to have only the U-shaped osteotomy (as described in the text) or in conjunction with double jaw surgery (so as a modified Lefort 1)? If the latter is the case, you could try to contact the first author of this paper ( https://www.ncbi.nlm.nih.gov/pubmed/29927828 ) who seems to practice in Mexico city (I do not know if this is easier for you than Spain, I did not find any other surgeons).

If you only want the U-shaped osteotomy without a Lefort 1 you can of course ask surgeons in your area if they perform it (which will be the only way, as nothing can be found on the internet).
However I would suggest you go with Alfaro as he published this technique and is generally regarded as a skissed surgeon.

Edit: I read in your other thread that you fear being treated bad if something goes wrong as a foreigner. I cannot comment on this doctor in Mexico city but I had a consult with Alfaro in Barcelona.
The staff and Alfaro himself were really pleasant as you described, too, and they seemed to have quite a few international patients.
Noone can guarantee you anything for any doctor but I would feel secure having it done there.

Thanks, haha I have already read this paper fortunately. I am looking to just get the U Shaped osteotomy, my jaws are not recessed by any means.
Probably would look better if it was moved forward a bit but not worth enduring that process.

Can you tell me more about your consult? Did you end up going for the procedure with him?
