Cosmetic/Functional advice, Surgery?

Hi, all i'm a male in his mid-twenties. I hate my profile (very receding chin and double chin despite the fact that i'm BMI:20 and bodyfat 12-14% -> a slim guy) since i was 16-17 (the problem started at 14) years old, i have done orthodontics from 11 to 14-15 and then i was followed up until 19-20 when i have extracted all my wisdom teeth.
I would like to fix the cosmetic issue but there also some functional problems: sistematic TMJ click during eating (60% of time), soreness if i eat certain types of food (i have to take a break to rest my jaw), sleep apnea(can come also from turbinate hypertrophy and slightly deviated septum), postural problems:forward head posture.
So after speaking with a general dentist he pointed me the fact the i have a too large overjet despite the fact that i have done orthodontics during the growing years and in his opinion the results obtained are not good all.
Then i came back to my old orthodontist who pointed out that my byte is acceptable and all my functional problems came from a swallowing issue and there are no skeletal issue from growing at all and said that probably i need to seek a mental specialist regard my cosmetic issue. I'm a bit skeptical about my old ortho opinion (i'm thinking that probably i'm a class 2 div 1 not masked very well...).
I'm going to consult other 2-3 ortho and a maxillofacial surgeon.

i have same issues with u. im very slim and but i have chubby cheeks too. forward head posture similar too. i think u need double jaw advancement because ur maxilla seem recessed too. Consult with maxfac surgeon first because ur bite not bad. And some orthos dont want surgery with good bites u can be get refused

In my country the social sanitary system pay for the surgery if you are elegible (functional issue) but there is a long waiting list (from 1 to 2 years) and you need a card that demand the first visit or i have to go in a private way.
My teeth are straight thanks to previus orthodontics (the start situation was drammatic XD) and fixed retainers on both jaw, the problem is the overbite.

my country same. If u have functional problem u can get everything but i have no. and i cant wait years for braces and surgerys. my previous orthodontics by general dentist and f**k them. my teeths straight but protruding.

Yeah you need bimax for sure - prob CCW rotation, or else large genioplasty, prob both.

I would like to fix the cosmetic part but a general dentist told me to not understimate the TMJ problem. I was thinking for a sliding genioplasty done from a top maxfac guy but in order to reach the lower lip i calculated that i need too much horizontal length.
For now i will seek some more opinion from other orthos and a maxfac.
I'm a little scared about the full surgery in particular regard to the paresthesia risk.

Orthos should tell the patient when skeletral discrepancy is severe but instead they prefear to do camufflage orthodontics with extraction in order to correct the byte partially and straight the teeth while destroying lower third face aesthetics.
