How risky is traveling abroad to get surgery?

Let's say from North America to Europe.

What happens in the event of complications, how much accountability will a european surgeon take? etc

I'd say it has a small effect on accountability. If a surgeon is going to ignore you they'll do it without taking your hometown into account. But they do want to keep a good relationship with their orthodontists since they'd risk losing their referrals if they routinely do a poor job. Since you'd use an unknown orthodontist you'd lose that little bit of leverage.

If there are complications then you would need to stay in Europe longer than anticipated.

I recommend against it. You want to be able to address any follow-up issues with clinical evaluation, and you lose that ability if your surgeon is on another continent.
