I think you could have a sliding genio which would kick up improvement to the chin. There might not be enough 'bone space' to make a full chin wing cut without risk to the nerve. But that's something the chin wing doctor would need to look at in terms of how close the nerve would be to the cut. So, I'd say sliding genio; vertically up and horizontally outwards. Might increase the crease/groove between lower lip and chin but that's not a bad trade-off in X-change for the better balance the genio would give. Also, as mentioned on this string, those kind of grooves can be filled in with filler.
