First of all I went to an ortho who worked together with my jaw surgeon but then I got a new ortho who didn’t know what he was doing and made camouflage treatment insted of decompensation. Fortunately no extraction took place. Now I found a new jaw surgeon and ortho but I think they are a bit conservative in terms of movements. They want to do double jaw surgery with genioplasty. I was told that advancing more than 7mm is not recommended on my lower jaw and that they would impact my maxilla only about 1mm. I guess it is meant to hide my gummy smile and for autorotation. Is that even enough for a noticeable change in my steep mandibular plane? I also have a narrow smile which I really want to be fixed but don’t know how. I’m also afraid that maxillary movement will caouse an upturned nose.

Other thing that really bothers me is that my right side profile is much weaker than my left. It’s quite noticeable from the front view, my left jaw angle is much angular. I also got a slight midline shift towards my right and the condylar head is flattened there as well. Is it possible that I have ICR?

Please take a look at my photos and share your thoughts. How my jaw could be fixed, what movements would be necessary?

For sure have a small jaw, but your occlusal plane is relatively flat and upper jaw seems good. Why not consider just LJS with genioplasty?

Thanks for the reply! Yeah occlusal plane is not steep but my lower jaw is still quite downswung somehow. I also have this deep antagonial notch. I want upper jaw surgery for my gummy smile and also want to advance it a little as my midface around my nose looks a bit sunken.
