Good candidate for a BSSO/Genioplasty?
« on: December 29, 2016, 06:04:00 PM »
I've been insecure about my face as long as I can remember. It's asymmetrical, and it seems disproportionate to me. I've always wanted to work on camera but as a result of hating how I look I've instead shunned photography my whole life. I really would like to get a BSSO/Genioplasty. I think I would be a good candidate but I was curious what you guys thought.

I had an open bite when I was younger, and mouth breathed because of bad allergies. I had braces to fix the open bite, but my teeth still didn't line up that well. I started doing the Mike Mewing thing where you clench your jaw constantly several years ago, and it has seemed to cause my teeth to fit better. It seems like I'm really grinding down my teeth fast, though, and I feel like the changes are more caused by my rear molars being compressed or ground down than my jaw structure changing. I'm looking for a more permanent fix that will make me look better without destroying my teeth.

I don't know many alternatives to BSSO/Genioplasty for long time results. I wouldn't mind doing a BSSO/Genioplasty and then going back somewhere to get jaw implants done for a masculine look. I want to work on camera and my appearance is holding me back from following my dreams. I need a nosejob too because I have a badly deviated septum and I don't know what order I should get these procedures in, either.

Photos blurry. Bite looks like a compensated class iii edge to edge, which is bad for your teeth. Nose job after jaw surgery always.

I don't have any really high quality pictures because I don't own any good cameras. I attached a pic from when I was in high school that I cropped, if it helps. My phone is broken and when I get a new one I can upload more pictures.

As far as a class iii, I don't know. I can say that I used to have an open bite, but after braces my front teeth met when I closed my mouth. Now if I don't pull my lower jaw backwards so that my molars meet then my front teeth meet when I close my mouth. I can hold my lower jaw in a backwards position all day but it gets really uncomfortable; as I type this my jaw is pretty sore from doing it most of the day. It's the only way I really feel comfortable in public though.

In the first photos you look class 3, in the second photos you look class 2. I'd suggest seeing a doctor and getting a proper diagnosis as a first step.

It makes sense that if you grind your teeth down they're going to fit better (Mew is a bit of a quack, IMO), but then of course you'll have no teeth, and the face will likely shorten (the oldman/collapsed "Popeye" look).

Have X-rays done by a surgeon so you get a proper diagnosis.
Millimeters are miles on the face.

GJ, the first set of photos was from this past month where the second set of photos was from high school, I think when I still had my braces on. That was 6+ years ago. So that picture from the second post would be 6+ years old. I guess that would mean that I've changed over the years? My teeth did fit much differently when I was younger.

I was going to wait until after I start a new job to begin testing and everything, because I think it would be expensive. I don't have a good job right now. I just was hoping for feed back, and maybe a little hope. I've been pretty depressed recently and I really hate how I look.

Find an orthodontist in your area who does surgical cases by searching online.  Email the office and ask for a consultation.  It should be free and should include ceph xrays.

You shouldn't do that because it puts too much strain on your joints and may damage them over time.

You have a mild Class III long face which requires double jaw surgery with counterclockwise rotation to properly fix it.

It's an ingrained habit to do whenever I'm in public now, unfortunately. I'd like to have a jaw surgery done as soon as possible. Is the double jaw surgery my best option?

Without worrying about my bite, am I aesthetically a good candidate? Would this type of surgery help my jaw angles from the profile? Would a double jaw surgery would provide my midface with more volume? I have high cheekbones but they don't project forward below my eyes and I have scleral show because of it.

Jaw surgery doesn't improve gonial angles. The mandible may swing up some with CCW rotation however.

From looking at before/afters I was under the assumption that a BSSO would improve jaw angle. I guess I would need a followup with some type of jaw implant if I were to get the result I'm looking for, and just use BSSO to improve my smile.

I like the results which Dr. Sailer has achieved in various patients. The Gonial angle on this man changed a lot:

Sorry for the double post, but I found this and thought it was relevant. I might be able to achieve a reasonable improvement with this, I wasn't trying to change my angles too significantly.
