For all the flack Sailer gets (probably deservedly), I know a guy who recently had a bimax with him and had excellent result. He was pretty good looking before and after he looks like a male model.

I would argue it's more difficult to take good looking face and make it model tier than take someone with recessed chin, lower jaw and trash tier submental region and make significant improvement. Vast majority of good surgeon results have those features (recessed chin/lower jaw coupled with double chin/bad neck region). if you take patients with other aesthetic issues than the ones I just mentioned, results are typically meh.
