Input for recommended procedures?
« on: August 22, 2018, 12:57:26 PM »
Want to confirm if my current surgical plan is the optimal one.

Currently thinking:
- 5mm MMA
- 5mm increase in maxillary height
- 4mm increase in chin height
- chin advancement (1-2mm past the lips)
- chin widening (t-shaped g-plasty, to compensate for SARPE)
- SARPE (enough to bring cheekbones out)
- Alar stitch to keep nose from widening during SARPE

For SARPE, my lower teeth will have to be tipped buccally. I think the chin widening is necessary since SARPE will make my mid face wider.

In the second set of pics, I lowered my jaw ~1cm and moved it forward about 5cm. I think that’s a ballpark to as good as I can get with surgery.

Any thoughts?

SARPE will make your lower midface fuller not wider.  This is the only effect it will have.  Your chin is not narrow, it's recessive.  It looks narrow because it's "far from the observer".

That makes sense. I'll drop the widening genioplasty from the plan then.

increase in chin height and chin widening aren't needed. Your chin is already long. Your cheekbones seem okay to me as well. And your chin shold not surpass the lips. You need chin advancement with somekind of procedure and maybe lowering the golonial angle like 2 millimeter or something. Going too far with lowering will give you a box appearance, which is not aestethic. Your nose is a bit big, but personally i don't like rhino, results feminize men and mostly aren't good. Maybe a very small refinement. Going overboard will feminize your face and fck up proportions.
