Bimax with CCW + Chin Wing?
« on: May 26, 2020, 11:35:54 AM »
Hello JSF,

i would like to improve my profile next year with some sort of jawsurgery and would like to ask for advice. I know the advice is for free and i have not much to give in return, but maybe my case can help other forum-members with similar situations. As seen in the CT my jaw and chin is set back and weak, even though i have perfect occlusion. Would i be able to improve this with bimax including CCW (in the best case without braces) and possibly a CW? Could the border of the mandible even be to close to the nerve for a CW?

Thank you for any advice!! :)

Your bite looks good, but the occlusial plane is steep. That is likely why the recession. Imagine rotating that entire occlusion CCW. You see how the plane of your teeth would flatten more, and your jaws would come out. Very likely that's what you need.

What did the surgeon who took that record recommend?
Millimeters are miles on the face.

Well the story is a little bit embarassing. Basically i visited an ortho in the beginning of last year, who made this scan. He told me that usually my situation requires surgery, without going into detail what exactly the surgery would be. He just said it is brutal and risky and he has a better alternative, which is the growth appliance "AGGA", to "grow the jaw in a 3D-way". At this point in time there was not much information about agga out and the only information that was available was positive, so i agreed. Fast forward i am almost finished with the appliance and during this time a lot of people finished the procedure with negative results and the general consensus is that it only moves the teeth a little bit along the occlusal plane, which does nothing for a steep plane, in fact it can even lenghten the midface. Even though i am not completly finished i can confirm this and the aesthetic benefit is minimal. It did not talk with my ortho about a potential surgery afterwards yet, because i wanted an unbiased opinion.

Growth appliances on adults are not going to replicate BIMAX corrective surgery to the full extent bimax can correct. Not for those people who need significant displacements.
